Let's make a list here.

And Lia, I will probably need help with this. I am trying to keep it short though.
REALLY BAD, OUT OF CHARACTER DOCTOR WHO MOMENTS:
10000 BC - whole thing, pretty much, I refuse to watch it again or show it to anyone, I don't care that it was the first storyline.
The Edge of Destruction - Everyone had a bad moment because the TARDIS was in their heads, but seriously, the TARDIS is not that dumb.
(I will not include Power of the Daleks because I can forgive this as a regeneration episode and the first at that. Note that the Twin Dilemma gets no such pass.)
The Evil of the Daleks - making adorably "human" Daleks, which act like little children, then sending them right around to fight normal Daleks in hopes that they will wipe one another out, saying "A final end" as he watches them burn. Way to commit genocide with children, Two (and way to remind me of that by saying the same thing, Eleven, grr)
(I'm not going to include Three's insults to the Brig during the seventh season, because he IS righteously pissed off at the man, but I will note that they really bother me. Even more, Liz Shaw agreed with the Brig's action that triggered the near-enmity, but she sides with the Doctor in bashing the rest of UNIT all season, which makes her worse. Note: Harriet Jones and the Brig have a bit in common.)
The Sea Devils - The Doctor calls a nuclear strike to wipe out an alien species. I don't care that he tried diplomacy first, this was really out of character. Extra problematic in conjunction with the seventh season. I seem to remember him smiling as he did it.
(I really want to put something from Four in here, but the bad moments aren't exactly out of character and the out of character moments aren't exactly bad. However, I must include some groaning at the sets and the science which were truly awful at times. The Invisible Enemy?! Aaaaaaugh!)
Twin Dilemma - I'm taking Lia's word for this one, and may NEVER watch it, even if I do eventually watch Six in all his terror. He tries to kill his companion. What.
